In het derde deel van de Underworldreeks gaan we op zoek naar de oorsprong van de eeuwenoude bloedvete tussen de aristocratische vampiers, bekend als Death Dealers, en de barbaarse weerwolven, de Lycans. Lucian, een jonge Lycan, werpt zich op als leider van de weerwolven. Zijn doel is om alle Lycans te verenigen en zo in opstand te komen tegen Viktor (Bill Nighy), de wrede koning van de vampiers. Lucian wordt in zijn strijd bijgestaan door Sonja (Rhona Mitra), een knappe vampier. Samen willen ze de Lycans bevrijden van het slavenwerk waar Viktor hen toe dwingt. Er ontstaat een bloederige strijd op leven en dood...
