The analysis, structure, and modeling presented in this volume were developed independently by The SozoRock Foundation.This publication addresses the structural causes of rural health inequities through a governance-based model that integrates technology, workforce renewal, and performance accountability. Drawing on regional data, workforce assessments, and SozoRock® internal modeling, the framework demonstrates how coordinated governance and measurable system design can strengthen service delivery in medically underserved areas.The report establishes a replicable prototype-Access Day-as a governance-based mechanism for aligning patient engagement, academic participation, and public-health accountability. Using the SozoRock® Equity and Resource Platform (SERP) and Rural Equity System (SRES) architecture, it introduces a standards-based model for counties, providers, and universities to transition from episodic outreach to measurable system integration.Authored by Oluwabiyi Adeyemo, MBA, DBA Candidate in Strategic Management, with contributions from Jordan Hare, BSN, RN, the series provides a national framework for applying evidence-based design to workforce stability, telehealth equity, and institutional continuity.
