Polypharmacy, defined as taking more than three to five medicines, is by no means uncommon at the end of life. The prescription of far too many medicines that are unnecessary and/or ineffective is a cause for concern. GPs are called upon to spare terminally ill patients medicines that are pointless, burdensome and often cause more harm than good due to interactions. Ultimately, as the number of medicines increases, drug interactions almost invariably occur. Rather than prescribing mechanically, it is therefore necessary to ask empathetically about the patient's needs in order to avoid unnecessary burdens. Palliative care teams can provide valuable guidance to help doctors, patients and their relatives in the decision-making process.
