Management of Differentiated Microcarcinomas the Thyroid

The incidence of differentiated thyroid microcarcinoma (mCD) has increased significantly due to incidental diagnosis. Although classified as a carcinoma, its natural history is most often indolent, posing a challenge regarding the aggressiveness of its management.Objective: To evaluate the clinico-pathological characteristics and disease progression profile of mCD, and to harmonize treatment and follow-up protocols in light of the most recent scientific data.Results: The prognosis for mCD is excellent, with a near-zero disease-specific mortality rate (< 0.5%) and a locoregional recurrence rate of less than 8% for low-risk forms. Its management has changed radically. The 2025 ATA recommendations endorse therapeutic de-escalation. Post-treatment follow-up is now streamlined and personalized through dynamic stratification.Conclusion: Recent international guidelines definitively endorse a minimalist and individualized approach to care in order to preserve patients' quality of life while ensuring their oncological safety.
