Effect of SMART on MIH affected Permanent Posterior Teeth: An overview

Silver Modified Atraumatic Restorative Treatment (SMART) is an emerging minimally invasive approach for managing MIH affected permanent posterior teeth. MIH is characterized by qualitative enamel defects that increase susceptibility to hypersensitivity, post-eruptive enamel breakdown, and dental caries. The porous and fragile enamel associated with MIH often compromises the longevity of conventional restorative procedures.SMART combines the caries-arresting and antibacterial properties of silver diamine fluoride (SDF) with the sealing and restorative benefits of glass ionomer cement (GIC). Application of SDF helps arrest carious lesions, reduces bacterial activity, and enhances dentin hardness, while the subsequent placement of GIC restores tooth structure, provides fluoride release, and improves function.Studies have demonstrated that SMART effectively reduces dentinal hypersensitivity, arrests caries progression, and improves patient comfort in MIH-affected teeth. The technique is particularly advantageous in pediatric patients due to its simplicity, cost-effectiveness, and minimal requirement for local anesthesia or extensive tooth preparation.
